How to register

Practice Area

The practice accepts patients living within the boundaries of Abercregan, Blaengwynfi, Abergwynfi, Bryn, Cynonvile, Croeserw, Cymmer, Cwmavon, Duffryn Rhondda, Glyncorrwg, Pontrhydyfen and Tonmawr.

If you move outside of the Practice Area

Patients, who move outside the Practice area, will be asked to register with another Practice that provides services within that area.

Registering as a patient

To register as a patient,  ask at reception for a registration form, you will need your NHS number.  You will be given an appointment for a Health Check.

Changes in your circumstances

Please help us to keep up to date by informing us promptly of any changes in your circumstances, especially changes in name, address and contact number. 

Temporary Residents

If you need to access general medical services and are away from home, you may register with the practice as a temporary resident.
